Tapes mislaid

With any prudent backup strategy, they should have been able to reconstruct most or all of the missing data from records held elsewhere.

Seems to me not just a case of remarkable human error, but remarkably poor system design that allowed human error to matter so much. I mean, human error is always with us and should be allowed for. Furthermore, tapes can be destroyed by natural disaster without need of human error.
